Bring your completed Request for Live Scan Service form provided by the requesting agency and a valid, acceptable government-issued photo ID. Make sure the information on your form is complete before your appointment.

Sperling Services charges a $40 rolling fee per applicant, plus applicable DOJ, FBI, agency, and transaction fees. Because government fees vary depending on the type of background check requested, the final total may vary. Mobile travel and scheduling fees may also apply.
